How to Book an Appointment

- Before clicking Okay, write down the VITA clinic address for your own reference.
- Select VITA Service
- Select IC UB
- Make your appointment
- Before your appointment, please print and fill out the intake sheet Form 13614-NR as best as you can, especially the top parts of each page, and bring it to your appointment as well.
Documents to bring with you for the appointment:
- Form I-20
- W-2 or any other tax forms you may have received
- Copy of any previous years' tax forms
- A list of the dates you have entered or left the United States during the entire period of your stay here
- Your check book (from a local bank)
- Your home country address
- Print and fill out the Form 13614-NR intake sheet in the link below
